It started with Victoria, and Ash and Shievonne continued with Catherine after Victoria left.I think Ash is to stupid to even realise she is not called Catherine but Caroline .

Quite a few of the HMs have called her 'Catherine.' I suspect it's her real name. Perhaps Caroline is her middle name? Or perhaps it's just an inside joke?

Victoria actually started it. However Ash is not the smartest person and probably forgot her name

Victoria kept calling Caroline "Catherine" because she didn't know her name and didn't bother to get to know her name. So it just stuck after that.
