They won't. Spencer has gone on record repeatedly that he never wants kids, and if Heidi does anything to get pregnant without his permission he'll divorce her. The only way Heidi is ever having a baby is if someone does an intervention, sends her to counseling and deprogramming, and she marries someone who isn't a sociopath.
