Best tactic I find is to place both your index and forefingers over the left button of you mouse. Then use these fingers alternately to tap the button (really fast). To maximise distance, start your tapping so that the needle hits a 45 degree angle.

For the very best result - always choose to boot Charley.

Edit to add: You may want to use your free hand to support the mouse and prevent it from running away from you.
