Sally Kellerman passed away at 84 after a long battle with dementia
Played the original Margaret 'Hot Lips' Houlihan in the series M*A*S*H
Her iconic career spanned over 150 movies and TV shows
She passed at an assisted care facility in Woodland Hills, California
