I think it was Daniel that was questioning if her boobs were real so he is aware of something being not quite right.

Certainly when viewed from the rear waist down she looks like a man to me.

There are men that look feminin and women that look masculin but you can't assume by looks alone that such people have had a sex change.

In terms of mannerisms I think Nadia is quite convincing.

I think it should be fairly easy to realise that she was not born a woman. Her (lack of) waist, her legs, her face, her voice...

I think that when Nadia wears her hair around her face, it softens her features a bit. However, when she was sitting in the diary room chair doing the live task on Saturday, you could see the masculinity of her features fairly clearly.
