..I've always thought it as referring to father but I have on a few occasions heard a few females say it about their OHs..not many though...I think more 'my old lady' referring to a spouse rather than a mother...

I think the term can be used for your father or your husband. The Song 'My Old Man's a Dustman' is clearly referring to his father, whereas in the song' Don't Dilly Dally', the line 'my old man said follow the van...' refers to her husband. Neither are particularly pleasant.
