I found that phrasing really weird (surely it would be "ER doctor" or something?) , and never got to the bottom of it but my theory was that she had a PhD , and also worked as an HCA.
Not saying I don't think she couldn't have been a doctor, more than happy to have egg on my face there - as should people doubting Matty's doctorhood! But with his age and general Mattyness, I'd lean toward "honorary bought PhD who works with the elderly :joker:
