Yep, he couldn't work because he had TB and that's why he couldn't pay off his debt... Arthur beat the guy literally to death and got his blood all over his face, in his eyes... Mouth... :umm2:.
There is a scene late game when Arthur is talking to either the priest or the nun... I can't remember which, it's one of them... But he's getting all philosophical about the things he's done and mentions something along the lines of that he can't feel too sorry for himself for having TB, because he caught it beating a guy to death for a few dollars.
It is an optional/random scene though I think so it's not actually made clear in every playthrough! It never comes up in any of the main quest cutscenes.
