There's almost always some sort of significant childhood event/events with true paedophilia, not necessarily a family member, can be another adult with access.

That said it sounds like this is underage teenagers rather than pre-pubescent children which isn't necessarily the same thing. That's not to downplay the severity, but there is a difference in the psychology.
