It does look photoshopped but it's possible to stretch the labret like that. I like the medusa piercing though.

its real as far as i can tell, the reason why the top teeth are showing is because the ring is pushing her lips upwards

You need to remember its pushing her bottom lip up. and even if you feel below your bottom lip you can feel your top teeth anyway
