Katie Holmes, who starred in Batman Begins, was offered the chance to reprise her role in The Dark Knight, which was critically acclaimed and went on to gross over $1 Billion, and turned it down to star in Mad Money, which is just an average comedy film that cost more money to make than it grossed.

Nicolad Cage for all of his films, minus Face Off.

He has made a few good films lately (well, 2 at least - Argo & The Town)

Michael Madsen for turning down the part of Vincent Vega in Pulp Fiction

Katie Cassidy leaving Supernatural for Harpers Island that lasted 16 episodes & Supernatural is in it's 8th season

Tom Selleck had to turn down the part of Indiana Jones, as shooting conflicted with Magnum P.I.
Not much he could do because of his contract, but still a pretty big role to turn down.
