When someone is banned...
 

If they are banned forever does it say 'Never' on the date their ban is up or does that mean at the mods/admin are having a discussion?

appaz the mods don't permaban anymore they just ban people indefinitely

Depends. A few that have been banned forever have eventually been allowed back.

Forever says never...but never tends to mean indefinite these days. Though some will never get another chance due to how horrific their behaviour actually was and the little/no chance they would change
