Are the teams always men v women?
 
Last night's was the first I've seen, do they mix it up as the evictions happen?

They usually mix them up after the first person leaves.

Most series (all but series 11, I believe?) start out as men vs. women, but the teams quickly get shuffled and usually change on an episode-to-episode basis
