It did come from the popes account, they are clearly blaming Instagram claiming it to be some sort of error.

Most likely scenario is that a member of Vatican staff was having a little instaperv and accidentally hit like while scrolling. Wups.

Obviously no one is going to admit that so they're like "We must got HACKED omg".

It's like the teenage boy caught with porn equivalent of "the dog ate my homework". Haha. "I waznt looking at boobs mum honest I got hacked by the bigger boys at school!"

Is it more likely that an Instagram staff member did it deliberately as a joke risking getting fired or possibly even sued for a quick laugh... Or that a sexually repressed junior Pope staffer tapped the wrong part of the screen whilst polishing the Vatican silver?

If the Vatican had just outright said "a junior member was in his wrong account" or something it'd probably be forgotten fairly quickly, but claiming they were hacked makes them look dodgy.

Probably just trying to make him look like a normal bloke as he fiddles kids on the side.
