If you look at the awards ceremonies where Ricky Gervais has been roasting people, he tends to mostly "punch up" (or seemingly up, it's more sideways as he is also very rich
) but he's usually taking aim at their wealth/status/how seriously they take themselves etc... I can even see him taking the piss out of DELIBRATE style/image choice... but I don't think he'd comment on something that was a result of a medical condition. Like you *can* do it but you'd only ever do it with someone you know very well, and who you are very sure will find it funny. e.g. I have a friend who has Type 1 diabetes, I've known him forever (since long before he was even diagnosed) and we've ALWAYS made "diabetes related" jokes. But I wouldn't make the same jokes with a random aquaintance or co-worker
