The %'s are wrongly presented.
If you claim a headache, you need to provide evidence of it via an official doctors notice, and the Dr needs to testify him/herself that it greatly affected the work rate - somethng 99% of doctors outright refuse to do because it questions their professional interest. Even if your doctor DOES do this, AQA need to examine the case and more often than not your request is declined.
