I'd imagine it'd be the same as most other cartilage piercings. The outer bit of the piercing is NOTHING, I had it pierced with a gun and is very, very common... painless to me.
The other side however may pinch, it's quite thick and tender there but it'll be over in a second. Just a crunch and it's done.
I was so, so scared getting my tragus pierced because mine is quite thick but I didn't feel a thing and have had no swelling, after-pain or complaints since and today changed from bar to hoop.
Honestly, go for it. It's over in a split second and tbh you won't REALLY know if it's "the most painful" until YOU get it done, everyone's experiences are different.
