You need to put only the text after the = in the [ yt ] [ /yt] things.

For example:

http://www.youtube.com/watch?v=Dn359Wh_YYw

Becomes this (with no spaces inside the []'s)

[ yt ]Dn359Wh_YYw[ /yt]
