I have never been asked to fill a form in and I have had all mine done at different places
Never been asked for ID either..even with my first when I had just turned 18
And yeah..just ask for what you want. I piss people off at this stage because I change my mind about 20 times and eventually the artist gets annoyed and says to just come back over when I have actually picked something
