Also an up to date rabies shot and before you re-enter the UK, your dog has to of been wormed (I think within 48 hours) by a vet.

We never get asked for any documentation when leaving the UK or entering another country but when you are coming back to the UK you have to park your car up, (in whatever country you're in) and take your dog into an office where they carefully scrutinize its documentation. They are really hot on the microchip documents being properly signed off and so anyone taking their dog abroad, make sure its been fully signed or your dog won't get back in.
