I have three cups of coffee before I head out to work in the morning. On the way to work I stop off and buy a large Americana with an extra shot and then drink another half dozen whilst at work. Once home I'll probably down two or three more coffees before going to bed. I'm probably swallowing around. 3 times the considered safe dose
Looks like I need to be buying decaf

Some people are more caffeine-sensitive than others... if you've been consuming that amount regularly with no issues, it's highly unlikely that it would suddenly be a problem. Also;

Schoolmates at Spring Hill High had said the 16-year-old had "chugged" the large bottle of energy drink before becoming ill during a lesson.
The final drink is believed to have caused the cardiac arrhythmia (irregular heartbeat) which led to his death an hour later.
Stressing that it was the speed at which the caffeine was drunk rather than the actual amount, the coroner said "the same amount of caffeine on another day may have been right"

This is the important part, really. My guess would be that he downed one of those large 750ml bottles of Red Bull style energy drink, and it's the speed of consumption that's the problem rather than the amount... his body simply couldn't process it quickly enough.
There's also very little testing done on how these drinks affect non-adults and they're strongly marketed at teens, which is a problem.
