Sometimes it's just simulated, I've read that in a few films the actors actually have sex.

It must be a difficult thing to do, as an actor myself, I'd probably keep laughing and have to re-take so I do respect actors that can do it.

In films (like Love actually) the man usually wears a pouch so he doesn't acidently invade private property. If it's shot from the waist up then the actors wear underwear. It must be quite cringey to do but I suppose they can block out the embarrassment being pro actors.
