Cocksucker is just a regular insult as far as I'm aware...no different to calling someone, for instance, a dickhead :S

I would consider shirtlifter to be a homophobic insult yes.

But its still more about context. If Brian himself wasn't offended by it, then there is no reason for others to feign mock outrage.

Umm not really. It depends on the context and how the person on the receiving end takes it. I would say anyway, others think differently.

Case in point Brian was offended when Jim Davidson called him a shirtlifter on Hells Kitchen.

I would see "Cocksucker" as a generic insult that is not meant literally
