If you're talking about the original chainsaw Massacre from the 1970's , that's probably why it's so effective. The hook scene feels more graphic than it actually is. I guess it's like the Psycho situation in the shower , implied violence is very strong. Horrific deaths but they're nowhere near as gratuitous as our minds think.

Nowadays there's so much OTT gore and it's always to gross us out or shock us for the sake of it.
