The Doctor was a title he gave himself because it's a name that means to do good and save people etc.

John Hurt's doctor wasn't one of the faces Clara saw because he somehow locked him away because he broke the promise that the name "Doctor" was supposed to signify. So either, he was the Doctor who fought in the Time War and committed mass genocide or as another poster suggested McGann fought in the war, resulting in a regeneration into Hurt who then (out of being born into war) did something terrible that the Doctor wants to block out.

So, effectively his "greatest secret revealed" wasn't his name like everyone thought but this missing Doctor in the form of John Hurt.

That's my understanding anyway.
