Rushdie wrote a novel that some religious leaders decided was an insult to their faith. The novel was a fantasy based tale and in part uses a dream sequence to introduce the story of Muhammed uttering disputed verses in the Qu'ran. He was also criticised by Christian leaders at the same time.

The Fatwa stating it was the duty of muslims to kill him or die trying was issued by Khomeini probably as a tactic to reintroduce Iran back into the Muslim world as an authority on Islam, it had just finished a war with Iraq and was in need of some good publicity. The islamic revolution was faltering and it needed something to kickstart it again. hence the call for his death.

Fatwa's are merely an opinion, a statement of interpretation and arent necessarily binding. Depends who issued it, and and the form of Islam people adhere to.

Thats true, however Rushdie lectures at some university in the states now. he also now considers the threat more rhetoric than real. He still has police protection though.
