I found a site which lists all bad continuity (silly little mistakes) for many films.
Be warned: Once you read some of them you'll probably not look at the film in the same way again. XD

I noticed this one from Quantom of Solace when I watched it:

Continuity: In the scene on Haiti, before Camille gets on the boat, she talks with Dominic Green and has her hands on his shoulders. But the positions changes between shots from shoulders to upper arm and back again.

I also remember my Dad mentioned ages ago about one really really SUPER bad error in this cowboy film where someone was not able to speak because he had his tongue chopped off... but later in the film he casually LICKS HIS LIPS! It's so easy for something like that to go unnoticed because it's such a casual thing. xD

Yeah they have a goofs section for each individual film on IMDb, you wouldn't believe how many mistakes every film has!
