A personal nurse would be private i should think and for long journeys would cost a small fortune Probably a set fee per day.

they usually have escorts, but with a severe condition maybe a nurse is inevitable.....though ive never heard of it paid for by the state

Private ops are sometimes paid for by the state both here and abroad but not sure for this long term scenario tbh.
