2nd guess.

Spoiler:

Misery. It's easy in my top 10 films of all time. Kinda pissed off the category was just "horror". I see it more as a thriller. The "horror" only threw me. Had it said "thriller" i'd have got it first time. I've seen the film probably 30 times.
