Her departure from the programme-making arm ITV Studios comes a year after the arrival of former Royal Mail chief executive Adam Crozier, who is trying to revive the company’s flagging fortunes. He is known for his tough approach towards workplace bullying.

‘Two members of her team complained about her behaviour towards them,’ one source said. ‘The complaint initially went to human resources but when the complainants wanted to take it further, it went all the way to the top.

‘They opened Sarah Caplin’s file and discovered that there had been many complaints about her over the years that hadn’t been pursued, especially from junior members of staff who accused her of bullying them. But there is a now a new approach at ITV and it was felt that Sarah’s behaviour could not be ignored.’

Caplin and Rantzen set up the 24-hour helpline Childline – now run by the NSPCC – after they were moved by an episode of That’s Life! about children who had suffered sexual abuse. Rantzen was appointed chairman and Caplin vice-chairman.

Caplin went on to land a job as producer of the BBC consumer programme Watchdog in 1989. From there she rose to be series producer, then executive producer and finally series editor.

She left the BBC in 1994 when she was appointed series editor of ITV’s This Morning. But three months later, she left amid reports of vicious behind-the-scenes fights with husband-and-wife presenters Richard Madeley and Judy Finnigan.

Caplin then moved to ITV Studios, where she has spent the past decade making shows such as House Of Horrors, nominated for a Bafta in 2001, Cosmetic Surgery Nightmares, Too Fat To Toddle, 60-Minute Makeover and Staff From Hell.

She has also made an astonishing six programmes about the reality star Jade Goody: Jade’s Salon, Just Jade, Jade’s Wedding, Bride To Be, A Year Without Her and Jade Changed My Life
