We are heading for it.I wouldn't say soon though but it will happen.
I don't think the nukes would go off straight away.I think that would be a very last measure when one side is pretty much crippled and saw no other option.Hopefully common sense would prevail before that point though and a peaceful solution would be reached.
I think it will start in a hot conflict zone in the middle east and possibly stay in that zone for a good while until more countries got involved and started attacking each others land.
Operations like taking out naval ships before they reached the battleground possibly not far from their home countries would start to happen.Then it would spread to the mainlands.
Russia would almost certainly try and take eastern Europe as the war progressed and that would be an intense battle.Possibly the worst battles would be fought there.

Doesn't really matter, none of the big powers want WW3.

At worst we might be heading into another gulf war if the Syria situation continues to escalate and the US and Russia will possibly find themselves in a cold war that will never get hot.

The only other dangerous political situation I can think of is the Korean war but, at worst, that's likely to become a proxy war, if anything. None of those possibilities are likely to spin into a WW3 scenario.

Nah. There will be some tensions and fighting in Syria between the Russia/Iran/Hezbollah alliance and USA though I feel. They won't allow the Syria regime change without a fight.
