They do so with each other's permission and are aware of it beforehand.

I'm sure if you went up to your friend and jabbed her in the eye with a fork she wouldn't appreciate it, nor would anyone find it funny.

Yes, I have watched it. Most of it's set up to look spontaneous, but it's not.

The same way TOWIE and other shows like it are set up and filmed as though real/fly on the wall documentaries but is fiction and scripted.
