Well... That's not necessarily true... If someone is in hospital for, say, blood pressure or heart issues (maybe caused by smoking, ironically) then stopping cold-turkey like that actually COULD kill them. It's highly chemically addictive - some weird and wonderful things happen to the body of people trying to quit - such as blood pressure spikes, anxiety attacks, etc... If you're not in the best of health, best to wean off slowly, not just stop. Same goes for pregnant women who are very heavy smokers - they should cut it out ASAP, but that means within a few weeks of finding out, not just there in the spot. A bloodpressure spike would be considered more risky for the pregnancy than smoking itself.
Why it can't be outright made illegal: for one, the tax money, but moreso, because you can't release a highly addictive substance for hundreds of years as legal (and make money from it), wait until a huge chunk of the population is addicted, and then say "sorry its illegal now! off to jail with you, junkies!". The process of banning it outright would be long, and expensive.
More on the topic of the thread - they should just find the area of the hospital with the worst aesthetics - maybe where the medical waste is collected - and have a sort of prison exercise yard style "smokers cage" where they all shuffle around like zombies having their cigarettes, out of sight. I definitely agree that there's nothing more foul than people puffing away at the main entrance. Or worse - the entrance to the maternity unit. sigh!
