Default Stupid decisions actors/actresses have made
I'll start with one I don't think can be beaten.

Katie Holmes, who starred in Batman Begins, was offered the chance to reprise her role in The Dark Knight, which was critically acclaimed and went on to gross over $1 Billion, and turned it down to star in Mad Money, which is just an average comedy film that cost more money to make than it grossed.

Tom Selleck had to turn down the part of Indiana Jones, as shooting conflicted with Magnum P.I.
Not much he could do because of his contract, but still a pretty big role to turn down.
Just think of Indy with that 'stache...

Harper's Island was a planned miniseries.

I would say Will Smith, he turned down a Tarantino movie that's coming out for MIB3.
