I like to think of it as the brain thinking faster than the fingers can type: so there's bound to be a mix up now and then.

It could never ever be attributed to operator error. We all know these pesky keyboards are at fault too, in not being able to read our minds, and do away with the need for the finger tapping. Bad keyboards!
