I can answer the English in foreign countries question - the teachers mostly go to America to learn English and it's easier to learn from American films and TV shows like How I Met Your Mother, Two and a Half Men etc than it is to, idk, watch Coronation Street, so the teachers generally have American-ish accents and so do the kids, but because they're learning in their spare time by watching TV and using the internet to pick up English, they're not being taught in the same way that we are over here, hence the weird accents. They're not being taught completely by their teachers.
