It vanished after i clicked cancel a few times it is really buggy though some of the tabs you click after the t&c page are still blank as well.

They ask you stuff like 'what are the three most interesting things about you' and 'what's the worst thing you've ever done'. About 3 pages of questions like that and then you have to upload 3 photos.

There is an option 'I am under 18'
I suppose it asks you for your date of birth to determine your question
