Well it depends. If it's work and I am expected to address someone formally, I would insist on the courtesy being returned. Out of work... nah, first names.

i mix between formal and first name all the time at work, depending on whether the customer has a hard to pronounce first or surname.

First name terms with most of my doctors, nurses etc.
I do call them doctor though, out of respect. |
