He actually says some really decent things in his rambling but... it really is rambling. It almost looks like a vocalised stream of "racing thought" - like his thoughts have totally gone off the rails and he's just opening his mouth and out they all come. He definitely needs help but it seems that he flat out refuses medication and prefers to ride out these episodes... which at the end of the day is his choice... it just has a pretty dramatic impact when it's someone who actually has a large public platform.
