I’m not an expert or anything, but I would say it’s to do with the brain and the mind. I guess the brain is telling a trans person that they are female even though their body does not conform to that message. Consequently, there is a permanent consciousness that their bodies are wrong, and often a hatred or disgust of anything in themselves that is typically male or masculine. Having watched a number of documentaries about the topic, children often start expressing this belief from a very early age (and I mean about 4-5).
When a guy is merely more effeminate rather than trans, it is a just a natural behavioural trait, but inside the guy knows he is male and feels like a man. He doesn’t question his body or have any doubts about it being right for him.
This is how I understand it anyway, but I might not be totally right because, as I said, I’m not an expert. I guess the best people to explain this would be trans people themselves, so hopefully there’s someone in the chat who might be able to offer a more accurate explanation. But hope that kind of helps.
