Surely if you pm the mod who gave you an infraction they won't reverse it or bother looking into it or do they have to open up a mod only discussion for other mods to chat about it?

I've reversed infractions I gave due to misunderstandings lots of times

This is what happens. Each infraction generates its own thread in the admin section. Even without someone PMing about them sometimes another mod will question it (or just say its too harsh/lenient) and a discussion occurs in the thread. When people PM me about infractions I usually tell them I will just post a comment in the infraction thread about it...

We don't tend to reverse each others infractions without a discussion. I don't remember a time that has ever happened actually.
