It's definitely more diplomatic to say "I don't follow what you're saying, or I don't understand" rather than accuse someone of not making sense.
So yeah, you either get that some people might take offense even if you don't mean to cause offense - or you don't.
Social interactions can sometimes require a bit of finesse and most people can do with a bit of refining in certain situations. If you aren't capable of coping diplomatically with people who may be sensitive to certain things, then so be it.
